Is my thesis paraphrasing still plagiarism?

I am writing my undergraduate thesis in Environmental Science and got a 12% similarity flag on my literature review. My supervisor said I need to rewrite the section because I kept the original sentence structure, but I thought changing every third word was enough. I am using APA 7th edition and am terrified that my work will be rejected for academic misconduct.
